Our client setup process has been updated to improve efficiency and client experience.
This form is no longer in use.
Starting January 5, 2026, all new client setups must be initiated through Salesforce.
The New Client Form will now be accessible only via the email link sent when an Opportunity is marked Closed/Won in Salesforce.
If you no longer have access to that email and still need to fill out a form for a Client, please reach out to your Sales Solution Leader to send you the link visible on the Opportunity in Salesforce.
